The Cornell Scott-Hill Health Center is proud to be a Patient-Centered Medical Home (PCMH). A PCMH is a system of care that creates a strong partnership between you and your personalized care team. Your team will be led by your primary care provider and may include nurses, health educators, specialists or any other medical personnel who can help you achieve your health and wellness goals. As partners committed to you and your wellness, we'll work with you and your family to coordinate easy access to care. We'll help you select a primary care provider (if you don't already have one) and will coordinate your care using our electronic health records. We'll involve you in every step of your care plan and teach you how to self-manage chronic conditions (such as asthma and diabetes) in order to stay healthy.

The Cornell Scott-Hill Health Center is committed to providing the highest quality of care. That’s why most of our programs and sites are accredited by The Joint Commission. The Joint Commission accredits and certifies nearly 21,000 health care organizations and programs in the United States. Joint Commission accreditation and certification is recognized nationwide as a symbol of quality that reflects an organization’s commitment to meeting certain performance standards.

Individuals having problems related to the care rendered, or problems with the environment of care, are encouraged to speak with the Health Center’s Patient Advocate. If concerns cannot be resolved through CS-HHC, individuals may contact The Joint Commission's Office of Quality Monitoring to report any concerns or register complaints about a Joint Commission-accredited health care organization by calling (800) 994-6610 or e-mailing complaint@jointcommission.org.


Cornell Scott-Hill Health Corporation is a federally qualified health center that, along with its employees, directors and certain contractors, is deemed by the Bureau of Primary Health Care of the U.S. Department of Health and Human Services to be a federal employee for purposes of medical malpractice claims.
