Public Housing Primary Care Program
Connecting residents of public housing to health care services.
The Public Housing Primary Care Program is designed to help residents of public housing get their health care needs met. If you are a resident of Public Housing in New Haven, West Haven, or Ansonia, you are eligible for our services. Program staff also provide education on important health-related issues such as diabetes, asthma, domestic violence, parenting, and so on. Staff can also assist you with some transportation and case management services.
