State Street Counseling Services

State Street Counseling provides outpatient substance abuse and mental health treatment for adult New Haven residents as well as surrounding towns.

Referral Process:
Please call (203) 503-3660 for referrals. The following information is requested at the time of the initial phone call:

• Name of client
• Current address
• Current phone number
• Date of birth
• Reason for referral
• Social Security Number
• Insurance information
• Name of referral source

Eligibility:
State Street Counseling Services is licensed to provide mental health to individuals over the age of 18 and substance abuse treatment to individuals over the age of 16 years.

Insurances accepted:
Most insurance are accepted including SAGA, HUSKY and Title 19

Services available:
Assessments for substance abuse and psychiatric disorders Individual therapy

• Group therapy including relapse prevention, anger management, relationship issues, parenting
• Family therapy
• Psychiatric evaluations and medication management (in conjunction with individual therapy)
• Case management services (limited)

***Please note that at time of initial intake a picture ID is required.
