Northside Community Outpatient Services

Northside Community Outpatient Services provides substance abuse and mental health treatment for New Haven resident as well as residents from surrounding towns.

Referral Process:
Please call (203) 503-3470 for referrals. The following information will be requested at the time of the initial phone call:

• Name of client
• Current address
• Current phone number
• Date of birth
• Reason for referral
• Social Security Number
• Insurance information

Eligibility:
Northside is licensed to provide mental health treatment to individuals over the age of 18 and substance abuse treatment to individuals over the age of 16 years.

Insurances accepted:
Most insurance are accepted including SAGA, HUSKY, Title-19, and some commercial carriers.

Services available:

• Assessments for substance abuse and psychiatric disorders
• Individual therapy
• Group therapy including relapse prevention, anger management, relationship issues, parenting
• Family therapy
• Psychiatric evaluations and medication management (in conjunction with individual therapy)
• Case management services (limited)
