Medical Clinic Opens at Crossroads

Crossroads and the Cornell Scott-Hill Health Center have partnered to open a new outpatient medical clinic at the outpatient treatment facility of Crossroads, located in the West Hills neighborhood of New Haven.

The outpatient medical clinic will serve clients participating in residential or outpatient treatment at Crossroads. Gerald Kayingo, a physician assistant, sees patients on Mondays, Wednesdays and Fridays from 1:30 to 5 pm, conducting physical examinations and addressing the chronic and acute care needs of the clients.

The clinic was opened with the help of start-up funding from the State Department of Social Services with the goal of reducing inappropriate use of hospital emergency rooms. This partnership continues the collaboration with the Cornell Scott-Hill Health Center whose health educators have provided prevention education groups on HIV and tuberculosis, and HIV testing and counseling, for Crossroads clients.

Crossroads, a non-profit agency offering outpatient and residential treatment services, is directed by Dr. Miguel Caldera. Amethyst House is a 15-bed Women and Children's Program at 42 Howe Street; the 66-bed Men's Unit and 44-bed Women's Unit are located at 54 East Ramsdell Street. Crossroads operates an Outpatient Psychiatric Clinic for Adults, and provides outpatient addiction treatment in the 44 East Ramsdell Street facility, serving up to 120 individuals from New Haven.

According to Dr. Caldera, a major reason that Crossroads sought this collaboration with the Cornell Scott-Hill Health Center was to provide on-site medical services, eliminating the need to transport clients to community providers, maximizing the participation of clients in addiction treatment, and enhancing service coordination by establishing the Cornell Scott-Hill Health Center on-site clinic as the provider of the majority of primary care services to clients.

Hill Health is a federally qualified health center in its 40th year of providing care to low-income residents of the New Haven area. The Center provides services in 18 locations in New Haven, West Haven, Derby and Ansonia.
