Northside Community Outpatient Services

Northside Community Outpatient Services provides substance abuse and mental treatment services for New Haven resident as well as surrounding towns.

Referral Process:
Please call 503-3470 for referrals. We request the following information at the time of the initial phone call:

• Name of client
• Current address
• Current phone number
• Date of birth
• Reason for referral
• Social Security Number
• Insurance information

Eligibility:
Northside is licensed to provide mental health and substance abuse treatment to individuals over the age of 16 years old.

Insurances accepted:
Most insurance are accepted including SAGA, HUSKY, T-19, and some commercial carriers

Services available:
Assessments for substance abuse and psychiatric disorders Individual therapy

• Group therapy including relapse prevention, anger management, relationship issues, parenting, domestic violence
• Family therapy
• Psychiatric evaluations and medication management (in conjunction with individual therapy)
• Case management services (limited)
