Social Worker/ ESP Program
SUMMARY

QUALIFICATIONS

Education (that amount of formal education normally required to perform the position duties:


MSW from an accredited school of social work.  License eligible preferred.

Experience (number of years and type required to meet an acceptable level of performance:

At least one (1) year experience working with relevant population (internship may qualify as experience). Ability to work interdisciplinary team.

Patient Registrar

State Street Health Services

Summary:

Under the supervision of the Manager or designee, greets patients and identifies, registers and maintains records of Hill Health Center patients. Primary responsibilities include the organization, input and maintenance of patient computer data to ensure conformance to registry operational standards and to meet the users of registry data.

MINIMUM JOB REQUIREMENTS:

High school diploma or GED; at least 1 year of experience that is directly related to the duties and responsibilities specified. Prior experience in a medical/healthcare environment or customer service setting preferred. Candidate must have a professional and proactive attitude and demonstrate strong customer service skills with the ability to apply them to our patients, external clients and internal staff

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

Excellent customer service skills; Team player; Strong verbal and written skills; Ability to communicate effectively and in a confidential manner; Data entry skills (40 wpm desired); Computer skills and familiarity with Microsoft, Internet and E-mail

Special Skills:

Knowledge of insurance and billing a plus; Bilingual communication skills preferred.

Patient Registrar, Pediatrics

Summary

DUTIES AND RESPONSIBILITIES:

Greet patients and schedules appointments via the AS 400 system.
Makes appointments for x-rays, specialties and medical consultation at other institutions
Review of data for completeness and accuracy.
Collects payment from patients for service provided.


Qualification

High school diploma or GED; 1 year of relevant experience. Prior experience in a medical/healthcare environment or customer service setting preferred. Clinical Coordinator

SUMMARY

Under the direction of the Director of Behavioral Health, the Clinical Coordinator maintains responsibility for BHD site assigned. Responsible for administrative and clinical oversight, including clinical supervision and maintenance of funding and accreditation standards.
QUALIFICATIONS
Master’s or PhD degree in Behavioral Health from an accredited school (e.g. MSW, MA, MFT) preferred. RN with Bachelor’s degree or CADAC with Bachelor’s degree may be considered.

Experience

All applicable candidates must have at least five (5) years experience in the treatment of patient with psychiatric disorders and associated conditions, at least two (2) years experience in the supervision of clinical staff. Experience as Program Director or similar administrative functions desired but not necessary.

Clinician/Northside Site

Speech-Language Pathologist
SUMMARY

Under the direction of the Program Director, ESP provides treatment of ESP children with communication delays through intervention, individualized programming, ongoing assessment, skilled observation, recording and checklist usage. Provides supervision to aides and volunteers as appropriate.

QUALIFICATIONS

Education (that amount of formal education normally required to perform the position duties:

 

  • Master degree in Communication Disorder/Speech - Language Pathology from a program accredited by the American Speech-Language Hearing Association (ASHA).
  • For Supervising Speech-Language Pathologist – Licensure in Speech-Language Pathology, Connecticut State Department of Health; Certificate of Clinical Competency in Speech Language Pathology (ASHA).
  • Other - Credentials indicate readiness to begin Clinical Fellowship Year (CFY) for ASHA, Supervised Clinical Experience (SPE) for CT State Department of Health under supervision of above.        
Experience (number of years and type required to meet an acceptable level of performance):

 

Special Skills:

A demonstrated knowledge of cognitive, social, language, adaptive and motor development of children.  Familiarity with developmental approaches to early childhood interventions.  A demonstrated knowledge of individualized programming and curriculum development.  A demonstrated knowledge of approaches to family involvement and support.  Must demonstrate age specific/population specific competencies in area(s) of practice.  Access to vehicle and CT. drivers License.  Knowledge of Spanish desirable.

Summary:

Acts as an advocate for patients seeking and receiving care in the Center and interprets to other members of the Health team the needs and concerns of the patient. Full time position at State Street Health site, working with OB/GYN, ENT patients and other specialties. Per diem position at SCRC providing care to patients recieving inpatient and out patient rehabilitation services

Qualifications

Requires a CT LPN license and ability to speak and write English; Bi-lingual in Spanish a plus

Summary:

Qualifications:

Full time positon available providing assessment, diagnosis, treatment to students in School-Based Health Clinics in the New Haven school districts. Comprehensive physical exams, immunizations, ugent visit treatment and health education. Prescribes medication as needed. This is a 10-month, school year position with opportunity for continued full time work in our community health clinics during the summer months if desired. Valid CT APRN or PA License required. Some experience preferred; bi-lingual in Spanish helpful.

Community Health Worker

Summary:

Under the direction of the Nursing Supervisor, acts an advocate for patients seeking and receiving care in the Center and interprets for other staff members the needs and concerns of the patients. Directly assists the clinical provider (physicians and mid-level practitioners) during clinical sessions. Prepares patients for exams according to specific guidelines

Qualifications:

High school graduate or equivalent. Connecticut CNA Certification required with at least one year of clinical experience. Must be able to read and write in English; Bi-lingual in Spanish preferred.

Nuturing Family Network, Home Visitor

Summary:

Under direction of the NFN Clinical Supervisor, conducts home visitation to identified first-time families for the purpose of delivering Parenting education via the Parents as Teachers and other curricula; case management; support, referral and advocacy services. Meticulous documentation required.

Qualifications:

High School/ GED diploma required. Some college preferred, with major course work in social work, human services, early childhood studies or related field. Minimum of three (3) year prior work experience as an outreach worker, family advocate, home visitor, counselor, social worker or similar position. Bi-lingual in Spanish preferred

Internist, Ansonia, CT
Diagnose and provide non surgical treatment of diseases and injuries of internal organ systems providing care mainly for adults who have a wide range of problems associated with internal organs.

Develop diagnostic and therapeutic plans and evaluate patients on a daily basis. 40hr/wk, 8am-5pm

Medical doctor degree or equivalent, three years residency in Internal Medicine, and eligibility for Connecticut medical license required.

Hill Health Center seeks an experienced person, with a minimum of a bachelor’s degree, to initiate and maintain various fund-raising activities, such as annual giving, major individual gifts, planned giving, corporate and foundation giving and organizing fund-raising events. Strong communication and computer skills needed. This position is responsible for implementing various fund-raising activities, including direct mail and events. Education Bachelor's degree.
