Engagement, Encouragement, & Empowerment

The HIV/AIDS Division provides a seamless continuum of care through a "one-stop-shopping" model of integrated HIV services. It is a client-centered organizational system built upon work teams consisting of multicultural and cross-trained HIV service providers. The Division’s Early Linkage Component provides immediate access to all Hill Health Center HIV Services at any point of client entry: prevention, medical, or social. The operating principles which govern services to our clients are Engagement, Encouragement, and Empowerment.

Primary Medical Services

The HIV Clinical Director, an Infectious Disease Specialist, leads the HIV team of highly experienced Primary Care Physicians in providing HAART regimes and best practice standards in the treatment of HIV across the disease spectrum. HHC provides a holistic and comprehensive approach to HIV treatment. PLWHIV/AIDS may access dental, nutritional, OB/GYN, and laboratory services on premises.

HIV Clinical Nursing Project

The HIV Clinical Nursing team acts as the linkage agent for patients to gain access to all primary and specialized services necessary in the treatment of HIV disease. The highly successful Medical Management Project offers intense and individualized patient education to support adherence to the complex medication regimes. Offering specialized nursing services, the clinical nurse acts as both encourager and caregiver to the HIV + patient. This vital partnership has proven to enhance patient engagement in medical care, thus producing improved treatment outcomes.

 

HIV Prevention and Outreach Services

HIV Prevention specialists provide specialized outreach and engagement services to high-risk substance abusers, women, and adolescences from greater New Haven’s communities of color. Services are designed to link consumers to collateral services, which reduce risks of HIV transmission and improve their quality of life, including: primary medical care, behavioral health, social services, etc. A special youth initiative, TAPA, has served hundreds of youth in a peer empowerment project initiated in 1993. All Prevention specialists possess Connecticut Department of Public Health HIV Prevention Certificates.

 

Services Are Offered With Respect To Client’s Culture And Lifestyle

HIV Counseling & Testing / Risk Reduction Services

Connecticut Department of Public Health Certified HIV Prevention Counselors provide state of the art HIV Counseling Testing Services. All consumers receive an individualized Client Risk Assessment to motivate and maintain behavioral changes that block their risk for HIV infection. Counselors link consumers through Prevention Case-Management to all services that support a positive, healthy lifestyle.

 

HIV Behavioral Health Services

HIV mental health and substance abuse specialists provide comprehensive services that integrate recovery from addictions, psychological support and self-empowerment to maximize consumers’ quality of life. Services are culturally and life-style sensitive with regard to each of the HHC PLWHIV/AIDS’s issues. Staff members hold appropriate CT licenses and credentials in the behavioral health field.

 

HIV Case-Management Services

Hill Health Center RYW Case-managers act as patient care advocates designing individualized Service Plans to support PLWHIV/AIDS across the disease spectrum. Case-Managers facilitate linkage to vital services: housing, medical, mental health, substance, AIDS Drug Assistance Programs, and RYW I & II Emergency funds. HHC Case Managers help the clients navigate the complex system of entitlements to ensure maximum health and financial coverage.

 

2002 Ryan White Care Grantee Conference- We Care. We Act.

Susan C. Campion LADC, LMFT Director of the Hill Health Center HIV/AIDS Division was a featured presenter at the 2002 Ryan White Care Grantee Conference- We Care. We Act., August 20-23rd in Washington DC. It is the largest national conference for all Ryan White Titles I, II, III,IV & and Dental HIV Providers. Over 1800 participants, HIV caregivers and consumers, attended the Conference with representation from every state and Puerto Rico. The Conference offered an array of workshops on current best practice standards for the provision of a continuum of HIV medical, behavioral health, and social services.

Ms. Campion presented the HIV/AIDS Division’s “Early Linkage Initiative” at the Workshop Session- In-Reach, Case Findings and Linkages Within Your System of Care. Betty Gramley, HRSA HIV/AIDS Bureau, the Workshop facilitator introduced the Hill Health Center’s linkage system located at a urban community health center as an “innovative and successful best practice model for swift access to an integrated HIV Service Network.”.  Click here to view this PowerPoint Presentation.
