2005 Hill Health Center’s Employee Weight Loss Program

Eligibility Requirements

Free to any Hill Health Center Employee who is overweight (BMI of 25 to 29.9) or obese (BMI of 30 or more) and who is ready to make lifestyle changes necessary for permanent weight loss. 

 

Rewards

$600.00 has been set aside to reward the 12 most successful participants (refer below for details).

 

Healthy Employee 2005:

Goal:  To promote healthy eating and physical activity among employees by giving pro-health information and providing motivational rewards and increased momentum through group participation.

 

Program Details:

  1. Healthy Lifestyle Changes - This program will motivate participants to make healthy lifestyle changes necessary for permanent weight loss.
  2.  Competition - Participants will receive points for making healthy lifestyle changes and may decide to compete against one another to see who can accumulate the most points.
  3. Accountability - A Weekly Participant Summary Form will be provided to participants to document changes and simplify tracking and reporting.  Each participant is expected to complete the Weekly Participant Summary Form and return it to the Program Leader on a weekly basis.
  4. Rewards - A reward system will be based on points that each participant accumulates throughout the program.  Points may be received for the following activities:

 

ü      Record keeping - Participants will receive 5 points for each day that food/exercise records kept. (Maximum per week = 35 points)

ü      Exercise - Participants will receive 1 point for every 15 minutes of exercise performed. (Maximum per week = 28 points)

ü      5-a-Day – Participants will receive 1 point for everyday that 5 or more servings of fruits and vegetables have been eaten. (Maximum per week = 7 points)

ü      Weight Loss - Participants will receive 1 points for every one-half pound lost. (Maximum per week = 4 points)

ü      Goal setting – Participants will receive 2-1/2 points per week that goals have been established. (Maximum per week = 2-1/2 points)

ü      Attending monthly educational presentations - Participants will receive 10 points per presentation attended. (Maximum per month = 10 points)

  1. Group Participation – Monthly educational presentations will be made available to participants throughout the program at the Main Clinic (400 Columbus Avenue Conference Room) from 12:30 to 1:15 pm.  In addition, off-site presentations will be made available if necessary (depending on group size).
  2. Honest Self-Reporting - Participants are expected to be honest in self reporting all activities.   Weekly weights should be obtained using the same scale.  The Participant Weekly Summary Form must be used to report all activities and weight loss changes.  Points will not be awarded unless the form is completed and submitted weekly to the Project Leader.  Individual progress will be updated and posted monthly on Hill Health Center’s Website.

Rewards:

1.      The top 12 participants who have accumulated the most points will be rewarded at a Healthy Employee 2005 potluck lunch. The prizes will be awarded based on the participants who have been most successful in achieving healthy lifestyle changes and weight loss.  The reward ceremony will be held in the 400 Columbus Avenue Conference Room.

Total Rewards = $600.00

 
1st to 4th Participants - $75.00 / person

5th to 8th Participants - $50.00 / person

9th to 12th Participants - $25.00/person
