New Elder Care Office Opens

 

 

Hill Health Center has opened a new Office of Elder Care for patients over the age of 60 years.  It is staffed by Sylvia Engelman, RN and Miriam Vitale, physician assistant, who are at 428 Columbus Ave., Monday-Friday, 8:30 to 5.   Call 203-503-3019 or 503-3010.

 

As a Federally Qualified Health Center, Hill Health offers sliding fee discounts and low-cost prescriptions.

 

The Center offers the following services to older patients:

* Geriatric assessment, including home visit, medication review, referrals to other agencies, mental health, activities of daily living (meals, bills, toilet, home safety).

* Diagnosis of Alzheimer's disease, caregiver support and resource information

* Other Medical services -

                        AIDS care

Dermatology (skin)

                        Ophthalmology (eyes)

                        Podiatry (feet)

                        Gynecology (women's medical care)

                        Internal medicine

                        Pain management

* Assistance with disability applications

* Dental

* Mental Health Counseling

* Nutrition Counseling

* Outreach

* Pharmacy

            Pharmacy Fund assists low-income people

* Preventive Care

            Diabetes

            Stop Smoking

            Screening for blood pressure, cholesterol

* Substance abuse treatment

* Free patient transportation

* Emergency food pantry

* Spanish-speaking interpreters

 

The Hill Health Center accepts most insurance, including Medicare, Medicaid, SAGA, ConnPACE, plus private insurance.
