Family and Medical Leave Request Information
All Managers and
Supervisors,
If
the need for leave is “foreseeable”, you should submit to your manager a
Family and Medical Leave Request Form (below) at least 30 days in advance of the
beginning of your leave. When the
leave is unplanned (e.g. medical emergency), you may give your supervisor verbal
notice, but you must complete a FMLA Request Form as soon as
practical.
Please be aware that when you are signing off on
Employees Time Sheets to be observant of their usage of sick days. Once
they have not return to work by the 5th consecutive sick day, please give them a
copy of the
F MLA form (attached) to
complete and return to the H.R. Dept.
