Attached are two Excel files: Diabetes_PatientCare_Summary.xls and Diabetes_MedcomLab_Summary.xls.
 
Diabetes_PatientCare_Summary.xls shows a summary of Activity in the PatientCare table of the Diabetes Registry system.  This shows that the only Activity entered into the registry for the period of your report (03/31/2002 through 05/31/03) is for Hemoglobin A1c. 
 
Diabetes_MedcomLab_Summary.xls shows a summary of Lab Results downloaded from MedCom for Patients with a diabetes diagnosis.  There does not appear to be any cholesterol values in this data so far.  (Please note that the MedCom data is incomplete because we only began continuous capture of this data recently.)
 
This should explain the values in your report.

Kristen checked the data I had sent to her, and confirmed that the data apprears to be importing properly.
 
It appears that, as far as the test results categories shown in the Diabetes Summary Report, there were only A1c test results imported for the period from 3/31/02 through 5/1/03.  I will look into this.
 
Regarding the patients with N/A instead of the provider name, I did find that we used the provider's social security number field instead of the provider ID field to find the provider's name.  We will create a Demographic file using the provider ID field which will result in many more matched.  We should then be able to update the PECS data.  I will let you know when this is done.  Sorry for the inconvenience.

I phoned Kristen regarding the report problem.  She will check to see if there are PECS issues related to the import process.  She did not realize that HHC is importing basically all the demographic, diagnosis and lab data.  I mentioned to Kristen that I have created a PECS detail report where lab results in addition to A1c show up.
 
I provided her with "anonymous" HHC data for testing.  Hopefully, she will get back within the next day or two. 
 
In the meantime, I will look into the provider issue.
 
I think it will be better to resolve the PECS import problem before we meet.  Until then, we could discuss issues by phone/e-mail.  Please let me know whether you think this is a good idea, or if you want to meet in any case.

Hi Bill , in response to your phone message - the person that I have had contact with through the Help Desk has been  Kristen diDonato at 302-235-7506 ext. 103.  The report that I ran was entitled the Diabetes Summary Report and chart #s 0000157,0004042, and 0007404 are examples of people that have provider n/a listed in PECS but a provider name in the Diabtes registry .  Hope this answers all your questions.  Let us know when you will be available to meet.  Thanks Brenda
